Marshfield Clinic is committed to your privacy as a visitor to our Web sites. Whenever the terms "we," "us," or "our" are used in this privacy statement, they refer to Marshfield Clinic. We collect limited information during your visit to our Web sites and only use such information to help us support and improve our relationship with you, our services, and your experience with our Web sites. We do not share the personal information that you provide to us with third parties.

Internet Protocol Addresses

An Internet Protocol address is a number that automatically identifies the computer or machine that you are using to access our Web sites. We collect aggregate information from our Web sites, including anonymous site statistics, by logging Internet Protocol addresses and/or domain names. This enables our server to send you the Web pages that you want to visit, as well as helps us diagnose problems with our server and administer our Web sites. We may track your session and use this information to tailor your Web experience. We use software called “LiveStats” to track site visits, page views and volume of traffic on our Web sites. LiveStats collects only anonymous user information in the aggregate. For more information about LiveStats, see www.deepmetrix.com/legal/privacy/products/.

Cookies

A cookie is a small data file that certain Web sites write to your hard drive when you visit them. A cookie file can contain information such as a user identification that the site uses to track the pages you visited. However, the only personal information a cookie can contain is information you supply yourself. A cookie cannot read data off your hard disk or read cookie files created by other sites. We use cookies when you log in as a user on some of our Web sites, and when you visit pages that are dynamically generated. We do not store passwords or any personal information about you in any cookie file. We use your user identification to identify you in our database and allow you to get access to restricted areas on our Web sites.

Forms That Request Personal Information

On certain pages, we give you the option of providing us with your name, address, phone number, email address, demographic information, preferences and other personal information. We only gather this personal information with your knowledge, permission, and participation. You have the option to limit the amount of personal information you provide us at the time you submit it.

Restricted or Members-Only Areas

We collect unique identification numbers assigned to you (such as medical history numbers) only to verify your identity when you visit restricted or members-only areas of our Web sites. We use member identification numbers only to verify that you are an active member (for example, a Security Health Plan member). We do not keep or use such identification numbers for any other purpose.

Links to Other Web Sites and Third Parties

Our Web sites may contain links to Web sites operated by third parties. While we provide links to these sites for your convenience, we do not control the persons or organizations who maintain these Web sites. We do not recommend or endorse the content of any third-party sites. We therefore cannot be held responsible for the content, accuracy or quality of information, or quality of services or products provided by or advertised on these third-party sites or the privacy practices of these third-party sites. We do not share unique identification numbers assigned to you for restricted or members-only areas of our Web sites with third parties.
